Your first visit to Macias Dermatology lays the foundation for our relationship with you, and we want it to be an easy one. During that visit we gather your current health information and medical history, so that we can offer an accurate, thorough diagnosis and a treatment plan suited to you.
Printing and completing the new patient forms before you arrive means your appointment can start on time. Bring them along with your insurance card and a list of any medications you take.
